Taina,
This part is not clear to me:
When you say - list the Illustrations publish and the number of incidences of publication, what time
frame are you referring to? If I had illustrations published in 1998, do I list those? my entire
work life, or is it just stuff that went out as first time or reuse in the two months at the
beginning of this year? If a image was sold for web use and is going to be up for 2 years, or for
infinity, how do those count?
Also, if the art is a true work-for-hire (ie a government contract) do those works count, or not.

What you put down is currently on the honor system, but it is subject to audit by ASCRL, so having the list ready is important. For us with big numbers, this is daunting. For this month, do a rough estimate. But very soon be prepared to make a list with solid numbers (so titles and ISBN numbers and number of illustrations).
