I use the tablet for everything, from web navigation to Photoshop. There is a mouse but I rarely use it (only to give my wrist a rest). I use the pen to select files or links to open, just like you would use a mouse. I don't use a "regular" USB mouse.

There is software that you install, after that the mouse or pen are what you use for selection and drawing.

So, I don't think it's the tablet size that causes wrist, arm or shoulder problems, but not paying attention enough to what I am doing or how I am doing it.
