As someone who has written test banks and surveys -- that survey was horrible and biased. However, it is nearly impossible to write a survey that is balanced and without bias of some kind. People's opinions are complex, and after you ask them one question you have influenced every answer after that. :) There's a whole profession dedicated to this sort of thing...
