I agree with Britt. Back in the day I heard to ask for 10x the one-time use. I tried it a couple times and got laughed at (hysterically, as in "It's April Fool's Day, right?"). 2x - 5x is more realistic.

As for name credit, I suggest you make sure you have that in a written contract. When it is a buyout in the sense of work-for-hire, the client usually will put their own name or omit it altogether sense wfh technically means you didn't do the art in the first place (though this is negotiable and I've had work for hire credited to me). When not work-for-hire I think it is whatever you both agree on.
